RESUMO

La cirugía de revascularización coronaria (CABG) es el estándar de tratamiento para la revascularización de la enfermedad de la arteria coronaria izquierda y/o de tres vasos. La cirugía coronaria sin bomba (OPCAB) evita el uso de derivación cardiopulmonar y puede mejorar los resultados a largo plazo al reducir las tasas de lesión miocárdica perioperatoria, accidente cerebrovascular (ACV), deterioro neurocognitivo y mortalidad de causa cardiaca. En la actualidad, se han llevado a cabo diversos ensayos clínicos desde la popularización del OPCAB en la década de los 90. Sin embargo, hasta el momento no se ha demostrado ningún beneficio del OPCAB en comparación con la cirugía tradicional a pesar de las reducciones favorables a corto plazo en los requerimientos de transfusión y otras complicaciones postoperatorias. Además, OPCAB se asocia con una revascularización miocárdica menos eficaz y no previene por completo las complicaciones tradicionalmente asociadas con la circulación extracorpórea (CEC). Este artículo revisa la evidencia actual de OPCAB en comparación con CABG tradicional en cuanto a los resultados clínicos a corto y largo plazo. Se analizan los resultados de la cirugía coronaria sin circulación extracorpórea (CEC) , comparándola con la cirugía convencional (con CEC). La revascularización coronaria sin CEC presenta resultados similares a la convencional, siempre que se cumplan determinadas condiciones en la selección de los pacientes. Una de ellas, muy importante, es la mayor experiencia del cirujano con el procedimiento.


The results of coronary artery revascularization performed without extracorporeal circulation (off pump) are compared to those of the traditional ("on pump") procedure. Compliance with selective conditions are required to obtain similar results. The most important being the experience of the surgeon performing the off pump procedure.

Introducción: Los tumores cardíacos primarios se caracterizan por su baja prevalencia, son principalmente mixomas y se presentan frecuentemente de forma asintomática. Objetivos: Identificar el tipo histológico más común, edad de presentación, tipo de cirugías y sobrevida de un grupo de pacientes tratados por Tumores Cardíacos Primarios (TCP) en el Hospital Regional de Temuco. Métodos: Revisión de fichas clínicas de 14 pacientes portadores de TCP entre marzo 2015 y diciembre 2021. Resultados: El tipo histológico más común fue el mixoma (85,7%), seguido por el fibroelastomas papilar (14,3%). La edad promedio fue 62 años (39-85), 9 fueron mujeres y 5 hombres. Los antecedentes mórbidos más comunes fueron: Insuficiencia Cardíaca Congestiva (ICC), Hipertensión Arterial (HTA) y Accidente Vascular Encefálico (AVE). La localización anatómica más común fue la Aurícula izquierda (92%). El tratamiento en el 92% de los casos fue resección aislada y en el 7% restante resección y reparación con parche. Conclusiones: Nuestros resultados son concordantes con la literatura.


Background: primary cardiac tumors are characterized by a low prevalence. Most of them are myxomas and asymptomatic. Aim: To describe the most common histological type, the age of presentation, type of surgery performed and survival of a group of patients operated on for Primary Cardiac Tumors (PCT) in the Hospital Regional de Temuco (Chile). Methods: Review of clinical records of 14 patients with PCT operated on between March 2015 and December 2021. Results: By far the most common histological type was a myxoma (85.7%), followed by a papillary fibroelastoma (14.3%). Mean age was 62 years (39-85), 9 were women and 5 men. The most common associated medical conditions were Congestive Heart Failure (CHF), Arterial Hypertension and Stroke The usual anatomical location was the left atrium (92%). Surgical treatment was isolated resection in 92% of cases and along with a patch repair in the remaining patient. Conclusion: good results were obtained, similar to those described in the literature.

Resumen: La oxigenación por membrana extracorpórea (ECMO) es de gran utilidad al proveer soporte ventilatorio a pacientes con hipoxia, pero su utilidad en el manejo de pacientes con obstrucción central de la vía aérea y riesgo vital no ha sido frecuentemente usada. La broncoscopía intervencional como terapia bajo ventilación convencional es de alto riesgo en este tipo de pacientes, pero es posible lograr excelentes resultados al ser asociada a ECMO. Comunicamos el caso clínico de 2 pacientes que presentaban disnea en reposo y falla ven1ila1oria aguda ca1as1rófica debido a una obstrucción casi total del lumen traqueal, de causa tumoral. En ambos pacientes en forma urgente se inició soporte circulatorio mediante ECMO VV, mientras se efectuaba la resección tumoral broncoscópica. Luego de terminada la cirugía traqueal, en ambos pacientes se retiró el soporte, siendo decanulados sin eventos y con una buena evolución clínica posterior. Se discute el beneficio del soporte ECMO en este tipo de pacientes.


Abstract: In patients with severe central airway stenosis bronchoscopy-guided intervention therapy under conventional ventilation conveys a high risk. Extracorporeal membrane oxygenation (ECMO) provides very good cardiopulmonary support, but is rarely used in bronchoscopy-guided interventional therapy. We report 2 patients with resting dyspnea due to severe tumor tracheal obstruction and acute pulmonary failure with imminent vital risk. Both patients were cannulated and the ECMO circuit installed on a nearly emergency basis. Tumors were excised, and the patients weaned from cardiopulmonary bypass uneventfully. Subsequent clinical course was satisfactory in both cases. A brief discussion of this condition is included.

RESUMEN: Se presenta el caso clínico de un paciente que presenta un infarto del miocardio con trombolisis no exitosa y posterior implantación de 2 stents coronarios quien desarrolla, algunos días después, una tormenta eléctrica ventricular. Una ablación de la taquicardia se realizó bajo ECMO, con buen resultado. Se detalla la descripción del caso, revisa y discute el tema.


ABSTRAC: A patient with a myocardial infarction whom, following a failed thrombolisis and implantion of 2 stents developed a ventricular electrical storm and hemodynamic instability. A successful ablation of the tachycardia with the use of ECMO was performed. A full description is included, along with a discussion of the subject.

Resumen Introducción: La cirugía de la válvula aórtica puede ser efectuada a través de una esternotomía reducida con menos trauma quirúrgico, una recuperación más rápida, mejores resultados cosméticos y excelentes resultados a largo plazo. Objetivo: Comunicar los resultados a largo plazo obtenidos con el abordaje mínimamente invasivo en el Hospital Regional de Temuco. Material y Método: Estudio de cohorte retrospectivo de 286 pacientes operados con técnica mini invasiva comparados con 1161 pacientes operados a través de esternotomía completa para reemplazo valvular aórtico por enfermedad valvular entre 2004 y 2019. La edad promedio fue 62,1±6,4 años. Para la canulación arterial y venosa se utilizaron cánulas de tamaño pequeño. El flujo de circulación extracorpórea promedio fue 4.5 L/min. Se utilizó cardioplejia sanguínea infundida por vía anterógrada y retrograda. Se utilizó estadística descriptiva con medidas de tendencia central y dispersión. Las variables categóricas fueron comparadas con el test de X^2 y las variables continuas con "t" test y ANOVA. La supervivencia actuarial se calculó mediante el método de Kaplan Meier con el programa Stata 14. Resultados: No hubo diferencia en la mortalidad a 30 días entre los grupos. Hallazgos similares se encontraron para arritmias, complicaciones cerebrales y bloqueo AV que requirió marcapasos. Hubo una diferencia estadísticamente significativa a favor del grupo de ministernotomía para reoperación por sangrado, días de estadía en UCI y estadía hospitalaria. El seguimiento promedio fue 101 ± 4 (1169 meses). La mayoría de los pacientes estaban en capacidad funcional I y libre de eventos cardiovasculares y reoperación. En el control post operatorio se hizo un ecocardiograma en el 86% de los pacientes mostrando buen resultado hemodinámico, similar en ambos grupos, y para distintas prótesis utilizadas. La probabilidad de sobrevida actuarial fue de 97% y la probabilidad de estar libre de eventos cardiovasculares fue 93% a 156 meses de seguimiento. Conclusión: La mini esternotomía es una técnica segura para la cirugía valvular aórtica con excelentes resultados en el largo plazo, permitiendo simplificar la cirugía, reducir los costos hospitalarios y permite una recuperación con menos secuelas cosméticas.


Abstract: Background: Aortic valve surgery can be performed through a reduced ministernotomy with less surgical trauma, improved cosmetics, faster recovery and excellent long-term results. Aim: To report the long-term results obtained with the minimally invasive approach technique in Southern Chile. Methods: A retrospective cohort study of 286 patients operated with the less invasive technique compared to 1161 patients operated on with full sternotomy for aortic valve replacement between 2004 and 2019 was performed. Arterial and venous cannulation was performed using small cannulas. Mean extracorporeal circulation flow was 4.5 L/min. Antegrade and retrograde cold cardioplegia was used. Statistical Analysis: Data are reported with measures of central tendency and dispersion. Categorical variables were compared with the X^2 test and ANOVA. Actuarial survival rate was calculated by Kaplan Meier method. The Stata 14 package was used for data analysis. Results: Demographic characteristics are shown in Table 1. Mean age of the whole cohort was 62.1 y/o (SD 6.4). There was no difference in 30 day mortality between groups. Incidence of arrhythmia, cerebral complications and AV block requiring pacemaker was similar between groups. There were no significant statistical differences in reoperation for bleeding, days of stay at the ICU and overall hospital stay. Mean follow up was 101 ± 4 (1169) months. After surgery most patients are in FC I and free from cardiac events and reoperation. In the postoperative period echocardiographic assessment was performed in 86% of patients showing similar hemodynamic function of the implanted valve. Actuarial survival rate was 97% and probability of freedom from cardiac events was 93% at 156 months of follow up. Conclusions: Ministernotomy is an excellent approach for aortic valve surgery which can be performed with very good results not inferior to those obtained with full sternotomy thus simplifying the surgical technique and reducing local hospital costs.

Resumen Antecedentes la reparación de la insuficiencia tricuspídea funcional está indicada en casos de insuficiencia cardíaca, generalmente, asociada a enfermedades de las válvulas cardíacas izquierdas y en algunos casos de enfermedad coronaria o cardiopatías congénitas. Objetivo : evaluar los resultados a largo plazo de la reparación tricuspídea en pacientes operados en la región centro sur de Chile. Pacientes y métodos : estudio retrospectivo de registros clínicos y operatorios en 114 pacientes con edad promedio 57.8 (DE 13) años (72 mujeres) sometidos a reparación tricuspídea asociada a otros procedimientos quirúrgicos entre 2009 y 2017. Resultados : la etiología de la insuficiencia tricuspídea fue debida a enfermedad reumática inactiva en 45% y a endocarditis infecciosa en 2.6%. 63% estaban en fibrilación auricular y 75% en capacidad funcional NYHC III. 15.8% tenían cirugía valvular cardíaca previa. El ecocardiograma mostró insuficiencia severa en 56% de los casos y había hipertensión pulmonar severa en 39.5%. La cirugía consistió en la implantación de un anillo semi-rígido en forma concomitante a reemplazo o reparación de lesión valvular mitral, reemplazo valvular aórtico, cierre de comunicación interauricular, cirugía coronaria o resección de tumor intra cardíaco. La mortalidad post operatoria global fue 16%, debido a falla multisistémica en 6%, insuficiencia cardíaca en 5% y hemorragia cerebral en 4.3%. El seguimiento promedio fue 78.8 (DS 7.2) meses. La supervivencia actuarial fue 74% a los 60 y 68% a los 96 meses. Conclusión : La cirugía de reparación de la insuficiencia tricuspídea moderada o severa, asociada a otras enfermedades cardíacas provee una buena recuperación sintomática, con excelente sobrevida alejada.


Abstract Background : Surgery for functional tricuspid valve insufficiency is indicated in cases of congestive heart failure usually associated to left heart valve diseases or as a concomitant surgery for other causes such as congenital or coronary disease. Aim : To assess and report the long-term results of tricuspid valve repair in adult patients in a regional cardiac surgery center in south Chile. Patients and Methods : Retrospective review of clinical and operative records of 114 patients aged 57.8 +/- 13 years (72 women) subjected to tricuspid reparative surgery concomitant with other cardiac procedures between 2009 to 2017. Results : In 45% of cases etiology was due to inactive rheumatic disease and 2.6% was due to endocarditis. 63% presented with atrial fibrillation and 75% were in NYHC CFIII. 15.8% had a previous cardiac valve surgery. Preoperative echocardiography showed severe tricuspid insufficiency in 56% of cases and pulmonary hypertension was severe in 39.5%. In all cases tricuspid repair was performed through the insertion of a semi rigid ring as a concomitant procedure for mitral repair/replacement in most cases, aortic valve replacement, surgical closure of an ASD, CABG surgery and the resection of cardiac tumors. Overall postoperative mortality was 16% due to multi-organic dysfunction in 6%, cardiac failure in 5% and cerebral hemorrhage in 4.3%. Mean long term follow up was 78.8+/- 7.2 months. Actuarial survival was 74% at 60 and 68% at 96 months. Conclusion: Surgical tricuspid valve repair for moderate to severe tricuspid insufficiency isolated or associated to other cardiac diseases provides a good symptomatic recovery, with an excellent long term survival.

Austrian syndrome is a triad characterized by pneumonia, meningitis and endocarditis, as a result of a Streptococcus pneumoniae bacteremia. We report a previously healthy 49 year-old male, who consulted at the emergency care unit with a history of one week of pleuritic pain, fever leading to an altered level of consciousness and seizures. A diagnosis of community-acquired pneumonia and meningitis was reached, isolating Streptococcus pneumoniae in the cerebrospinal fluid and blood cultures. Antibiotic treatment was started but the patient had an unsatisfactory response. During hospitalization a new heart murmur was found in the physical examination. An echocardiography was performed and a massive aortic valve insufficiency was found along with vegetations and a perforation of the same valve. The valve was replaced by a prosthetic one and the patient responded satisfactorily to the surgical and antibiotic treatment, without complications.

BACKGROUND: We previously reported that there was no significant difference at 30 days or at 1 year in the rate of the composite outcome of death, stroke, myocardial infarction, or renal failure between patients who underwent coronary-artery bypass grafting (CABG) performed with a beating-heart technique (off-pump) and those who underwent CABG performed with cardiopulmonary bypass (on-pump). We now report the results at 5 years (the end of the trial). METHODS: A total of 4752 patients (from 19 countries) who had coronary artery disease were randomly assigned to undergo off-pump or on-pump CABG. For this report, we analyzed a composite outcome of death, stroke, myocardial infarction, renal failure, or repeat coronary revascularization (either CABG or percutaneous coronary intervention). The mean follow-up period was 4.8 years. RESULTS: There were no significant differences between the off-pump group and the on-pump group in the rate of the composite outcome (23.1% and 23.6%, respectively; hazard ratio with off-pump CABG, 0.98; 95% confidence interval [CI], 0.87 to 1.10; P=0.72) or in the rates of the components of the outcome, including repeat coronary revascularization, which was performed in 2.8% of the patients in the off-pump group and in 2.3% of the patients in the on-pump group (hazard ratio, 1.21; 95% CI, 0.85 to 1.73; P=0.29). The secondary outcome for the overall period of the trial - the mean cost in U.S. dollars per patient - also did not differ significantly between the off-pump group and the on-pump group ($15,107 and $14,992, respectively; between-group difference, $115; 95% CI, -$697 to $927). There were no significant between-group differences in quality-of-life measures. CONCLUSIONS: In our trial, the rate of the composite outcome of death, stroke, myocardial infarction, renal failure, or repeat revascularization at 5 years of follow-up was similar among patients who underwent off-pump CABG and those who underwent on-pump CABG. BACKGROUND: Previously, we reported that there was no significant difference at 30 days in the rate of a primary composite outcome of death, myocardial infarction, stroke, or new renal failure requiring dialysis between patients who underwent coronary-artery bypass grafting (CABG) performed with a beating-heart technique (off-pump) and those who underwent CABG performed with cardiopulmonary bypass (on-pump). We now report results on quality of life and cognitive function and on clinical outcomes at 1 year. METHODS: We enrolled 4752 patients with coronary artery disease who were scheduled to undergo CABG and randomly assigned them to undergo the procedure off-pump or on-pump. Patients were enrolled at 79 centers in 19 countries. We assessed quality of life and cognitive function at discharge, at 30 days, and at 1 year and clinical outcomes at 1 year. RESULTS: At 1 year, there was no significant difference in the rate of the primary composite outcome between off-pump and on-pump CABG (12.1% and 13.3%, respectively; hazard ratio with off-pump CABG, 0.91; 95% confidence interval [CI], 0.77 to 1.07; P=0.24). The rate of the primary outcome was also similar in the two groups in the period between 31 days and 1 year (hazard ratio, 0.79; 95% CI, 0.55 to 1.13; P=0.19). The rate of repeat coronary revascularization at 1 year was 1.4% in the off-pump group and 0.8% in the on-pump group (hazard ratio, 1.66; 95% CI, 0.95 to 2.89; P=0.07). There were no significant differences between the two groups at 1 year in measures of quality of life or neurocognitive function. CONCLUSIONS: At 1 year after CABG, there was no significant difference between off-pump and on-pump CABG with respect to the primary composite outcome, the rate of repeat coronary revascularization, quality of life, or neurocognitive function. BACKGROUND: The relative benefits and risks of performing coronary-artery bypass grafting (CABG) with a beating-heart technique (off-pump CABG), as compared with cardiopulmonary bypass (on-pump CABG), are not clearly established. METHODS: At 79 centers in 19 countries, we randomly assigned 4752 patients in whom CABG was planned to undergo the procedure off-pump or on-pump. The first coprimary outcome was a composite of death, nonfatal stroke, nonfatal myocardial infarction, or new renal failure requiring dialysis at 30 days after randomization. RESULTS: There was no significant difference in the rate of the primary composite outcome between off-pump and on-pump CABG (9.8% vs. 10.3%; hazard ratio for the off-pump group, 0.95; 95% confidence interval [CI], 0.79 to 1.14; P=0.59) or in any of its individual components. The use of off-pump CABG, as compared with on-pump CABG, significantly reduced the rates of blood-product transfusion (50.7% vs. 63.3%; relative risk, 0.80; 95% CI, 0.75 to 0.85; P<0.001), reoperation for perioperative bleeding (1.4% vs. 2.4%; relative risk, 0.61; 95% CI, 0.40 to 0.93; P=0.02), acute kidney injury (28.0% vs. 32.1%; relative risk, 0.87; 95% CI, 0.80 to 0.96; P=0.01), and respiratory complications (5.9% vs. 7.5%; relative risk, 0.79; 95% CI, 0.63 to 0.98; P=0.03) but increased the rate of early repeat revascularizations (0.7% vs. 0.2%; hazard ratio, 4.01; 95% CI, 1.34 to 12.0; P=0.01). CONCLUSIONS: There was no significant difference between off-pump and on-pump CABG with respect to the 30-day rate of death, myocardial infarction, stroke, or renal failure requiring dialysis. The use of off-pump CABG resulted in reduced rates of transfusion, reoperation for perioperative bleeding, respiratory complications, and acute kidney injury but also resulted in an increased risk of early revascularization. BackgroundThe relative benefits and risks of performing coronary-artery bypass grafting (CABG) with a beating-heart technique (off-pump CABG), as compared with cardiopulmonarybypass (on-pump CABG), are not clearly established.MethodsAt 79 centers in 19 countries, we randomly assigned 4752 patients in whom CABG was planned to undergo the procedure off-pump or on-pump. BACKGROUND: Surgery for active endocarditis is indicated in cases of congestive heart failure (CHF), persistent sepsis, systemic embolization and paravalvular involvement. AIM: To assess and report the long term results of surgery in adult patients. PATIENTS AND METHODS: Retrospective review of clinical records and operative procedures of 32 patients aged 43+/- 13 years (28 women) subjected to reparative surgery for complications of endocarditis between 1993 to 2005. RESULTS: In 25% of cases, endocarditis presented as a prolonged sepsis syndrome and in 31% as a CHF or both. The causative bacteria was Gram (+) in 53% and blood cultures were negative in 47%. Preoperative echocardiography showed vegetations in 56% of cases. An annular abscess, aortic valve rupture and bicuspid valve, was observed in 13% of patients. Post operative mortality was due to persistent sepsis and multi-organic dysfunction in 16%. Mean long term follow up was 43.8+/-47.2 months. Actuarial survival was 78% at 146 months. CONCLUSION: Surgical management of active endocarditis provides a good symptomatic recovery, with an excellent long term actuarial survival.

BACKGROUND: The use of left internal mammary artery (LIMA) as a graft to anterior descending artery (LAD) has been associated with better long term results in coronary surgery. AIM: To assess and report the long-term results of LIMA to LAD bypass grafting for isolated LDA lesions. PATIENTS AND METHODS: Retrospective analysis of the medical records and surgical protocols of 40 patients (aged 60+/-10 years, 28 male) subjected to coronary surgery between 1992 and 2002. RESULTS: Thirty-four patients presented with unstable angina. On angiography, the LAD had a proximal obstruction in 35 patients. Sixteen presented with a myocardial infarction of the LAD territory. Six were managed previously with angioplasty; four had a new critical obstruction, 1 was catalogued as a procedure failure, and one was totally occluded. There was no operative mortality, myocardial infarction, stroke or need for re operation. There were two late deaths, caused by an advanced cardiac failure at 120 months in one patient, and chronic renal failure at 61 months of follow-up in another. Actuarial survival probability was 100%, 93% and 75% at 1, 5 and 10 years. Probability of freedom from angina was 98%, and freedom of suffering a new myocardial infarction was 100% at more than 10 years. The probability of no need for a new coronary procedure (angioplasty or surgery) also was 100% at more than 10 years. CONCLUSIONS: The use of LIMA as a coronary bypass graft to LAD is a safe surgical technique, with an excellent duration and permeability and also provides a prolonged time free from cardiac events as mortality, angina, myocardial infarction, and the need of a new coronary procedure.

Introducción: La cirugía de la comunicación interauricular (CIA) tiene excelentes resultados a largo plazo en niños y también en pacientes operados en la etapa adulta. Objetivo: Determinar y comunicar nuestra experiencia respecto de los resultados a largo plazo del tratamiento quirúrgico de la CIA en adultos. Pacientes y Métodos: Estudio descriptivo de 58 pacientes intervenidos entre los años 1991 y 2004. Cincuenta y seis tenían CIA tipo ostium secundum y 2 tipo seno venoso; 4 presentaron insuficiencia tricuspídea funcional. Veintiocho pacientes tenían hipertensión pulmonar moderada y 25 severa. El shunt promedio fue de 3.2 ± 1.016. Resultados: La edad promedio fue 31.8 ± 11.34 años; 42 pacientes eran de sexo femenino y 14 mayores de 40 años; 15 pacientes estaban en CF II y 43 en CF III (NYHA). En 56 pacientes el cierre del defecto se realizó con parche de dacrón, en 1 con pericardio autólogo y cierre directo en otro. En 3 pacientes se efectuó una anuloplastía tricuspidea y reemplazo en 1. Una paciente falleció por tromboembolismo pulmonar masivo postoperatorio, 3 pacientes presentaron fibrilación auricular transitoria y 1 paciente presentó un derrame pleural. La probabilidad de sobrevida actuarial fue de 98 por ciento a 176 meses y la probabilidad de estar libre de arritmias fue de 95 por ciento a 176 meses de seguimiento. Conclusiones: En nuestra experiencia, el cierre quirúrgico de la CIA en el adulto provee una excelente mejoría sintomática, encontrándose la mayoría de los pacientes en CF I, con una excelente probabilidad de sobrevida y tiempo libre de eventos cardiovasculares en el largo plazo.

La hidatidosis cardíaca tiene una incidencia de 0.5 a 2 por ciento, siendo la ubicación pericárdica la más frecuente. En los últimos años en nuestro hospital se han presentado 2 casos de hidatidosis intracardíaca (HI). Primer caso: varón de 15 años con diagnóstico de muerte súbita mientras realizaba ejercicio. La autopsia demostró un QH roto en músculo papilar de ventrículo izquierdo (VI). Segundo caso: mujer de 26 años con antecedente de cirugía por hidatidosis hepática. Ingresa a UCI por taquicardia supraventricular. El estudio por imágenes mostró tumor quístico intracavitario en pared inferolateral de VI. Las pruebas serológicas para hidatidosis fueron positivas. Se efectúa cirugía en julio de 1998, a través de esternotomía media bajo circulación extracorpórea, hipotermia y cardioplejía, practicándose quistectomía más capitonaje de la pared VI. La evolución postoperatoria fue favorable, no hubo complicaciones, siendo dada de alta al 8º día. El tratamiento de la HI debe ser quirúrgico con buenos resultados

RESUMO

Se evalúan los resultados quirúrgicos de la endarterectomía de la arteria coronaria descendente anterior, reparación con parche venoso y anastomosis de la arteria mamaria interna izquierda como procedimiento asociado a cirugía de revascularización miocárdica en 10 pacientes. Se evalúan las características clínicas de los pacientes. El estudio coronariográfico demostró lesiones críticas obstructivas y/o difusas de la ADA en todos los casos. Se describe en detalle la técnica utilizada, la cual se realizó en forma exitosa en todos los casos con mejoría de su capacidad funcional previa. La técnica utilizada es una alternativa quirúrgica segura en la reconstrucción de endarterectomías coronarias amplias de la ADA con baja morbilidad asociada
